Chronic Kidney Disease (CKD) is a chronic condition with a high mortality rate, particularly among hospitalized patients. Systemic inflammation plays an important role in the deterioration of CKD prognosis. The Neutrophil-to-Lymphocyte Ratio (NLR) is a simple, inexpensive, and readily available inflammatory marker obtained from a routine complete blood count, making it a practical potential predictor of mortality in CKD patients. This study aims to determine the association between NLR and mortality in hospitalized CKD patients at RSUD Waled Cirebon in 2025. This analytical observational study utilized a cross-sectional design based on medical record data. A total of 30 subjects were selected through consecutive sampling according to inclusion and exclusion criteria. The NLR cut-off value was set at 5.00, consistent with previous studies. The primary analysis employed the Chi-Square test, confirmed by Fisher's Exact Test, alongside the Mann-Whitney test and Spearman Rank correlation as supplementary analyses. Out of 30 subjects, 13 patients (43.3%) died and 17 patients (56.7%) survived. The proportion of deaths in the high NLR group (66.7%) was considerably higher than in the low NLR group (20.0%). A significant association was found between NLR and mortality (Chi-Square p=0.010; Fisher's Exact p=0.025), with an Odds Ratio of 8.00 (95% CI: 1.52–42.03). The Mann-Whitney test showed significantly higher NLR values in the deceased group compared to survivors (p<0.001), and Spearman Rank correlation showed a positive, strong association between NLR and mortality (r=0.618; p=0.000). In conclusion, there is a significant association between NLR and mortality in hospitalized CKD patients at RSUD Waled Cirebon in 2025.
