Abstract

Background: Tuberculosis (TB) remains a major public health problem in Indonesia, which ranks among the countries with the highest TB burden globally. Medication adherence during the six-month or longer treatment period is a decisive factor for cure, and patient knowledge is often postulated as one of its determinants. Objective: This study aimed to describe the level of knowledge and medication adherence and to analyze the relationship between the two among pulmonary TB patients at PB Selayang II Community Health Center, Medan City. Methods: This was a cross-sectional analytical study on 36 pulmonary TB outpatients selected by purposive sampling from July to December 2025. Data were collected through a validated 22-item knowledge questionnaire and the Morisky Medication Adherence Scale (MMAS-8). Univariate analysis used frequency distributions; the bivariate association between knowledge and adherence was analyzed using the Fisher-Freeman-Halton exact test because more than 20% of expected cell counts were below five. Results: Most respondents were male (25 ; 69.4%), aged 20–40 years (20 ; 55.6%), had senior high school education (22 ; 61.1%), and were unemployed or retired (13 ; 36.1%). Knowledge was categorized as high in 19 respondents (52.8%) and moderate in 17 (47.2%), with no respondent in the low category. Medication adherence was moderate in 27 respondents (75.0%), low in 7 (19.4%), and high in 2 (5.6%). The Fisher-Freeman-Halton exact test showed no statistically significant association between knowledge and adherence (p = 0.435). Conclusion: Although the majority of pulmonary TB patients at PB Selayang II had moderate to high knowledge, medication adherence remained predominantly at the moderate level, and no statistically significant relationship was found between knowledge and adherence in this sample. Strengthening pill-count monitoring and Directly Observed Treatment (DOT) may still be warranted to improve adherence.
