Analisis Korelasional Skor Child-Pugh dan lama Perdarahan pada Pasien yang Menerima Terapi Oktreotid di RSUP Dr.Kariadi

Gastrointestinal bleeding due to esophageal variceal rupture is a life-threatening complication of liver cirrhosis. Octreotide is widely used as a first-line pharmacological therapy to control bleeding. However, bleeding duration varies among individuals and may be correlated with liver functional reserve. This study aimed to analyze the correlation between Child-Pugh score, its constituent components, transfusion characteristics, and bleeding duration in liver cirrhosis patients receiving octreotide therapy at Dr. Kariadi Central Hospital. A cross-sectional analytical study was conducted involving 80 liver cirrhosis patients with esophageal variceal bleeding treated with octreotide. Data on demographics, Child-Pugh class, constituent clinical/laboratory parameters, transfusion types, co-medications, and bleeding duration were recorded. Data non-normality was established, and Spearman's rank correlation test (Spearman's rho) was applied to evaluate the correlation between Child-Pugh score and bleeding duration. Patients were predominantly aged 40–60 years (58.8%) and male (72.5%). Most patients belonged to Child-Pugh Class B (56.3%), followed by Class A (23.8%) and Class C (20.0%). Packed Red Cells (PRC) were the most frequently transfused blood component (73.8%, mean 3.59 units). The overall mean bleeding duration was 2.93 ± 1.92 days, with Class A averaging 2.53 ± 2.04 days, Class B 2.76 ± 1.43 days, and Class C 3.88 ± 2.68 days. Spearman's correlation analysis revealed a statistically significant positive correlation between Child-Pugh score and bleeding duration (r = 0.252, p = 0.024). There is a statistically significant positive correlation between Child-Pugh score and bleeding duration in liver cirrhosis patients receiving octreotide therapy. Higher Child-Pugh scores are associated with prolonged duration of bleeding.
