Abstract

Chronic rhinosinusitis (CRS) is a persistent inflammatory disease of the paranasal sinus mucosa lasting at least twelve weeks. Its non-specific presentation frequently causes diagnostic delay, worsens patient outcomes, and increases healthcare costs. This study aimed to analyse the association between patient characteristics and paranasal sinus CT scan findings with delayed diagnosis of CRS at Mitra Medika Bandar Klippa General Hospital in 2026. An observational analytic study with a cross-sectional design was conducted on 120 patients selected through consecutive sampling from January to December 2026. Data were obtained from medical records and paranasal sinus CT scan reports. Bivariate analysis employed Chi-Square and Fisher's Exact tests with a significance level of p<0.05, and Odds Ratios (OR) with 95% Confidence Intervals (CI) were calculated. Delayed diagnosis was identified in 56.7% of patients. Variables significantly associated with delayed diagnosis were age ≥40 years (OR=2.38; p=0.024), low educational level (OR=2.46; p=0.030), smoking status (OR=2.96; p=0.006), history of allergy (OR=2.39; p=0.022), duration of symptoms ≥12 weeks before consultation (OR=4.45; p<0.001), Lund-Mackay score ≥12 (OR=3.46; p=0.001), presence of nasal polyps (OR=4.20; p<0.001), and bilateral sinus involvement (OR=2.78; p=0.009). Duration of symptoms and nasal polyps demonstrated the strongest associations. Strengthening early detection, patient education, and timely referral pathways for CRS is recommended to reduce diagnostic delay
