Occupational accidents remain a significant concern in the industrial sector, as most incidents are associated with unsafe work behavior (unsafe actions). Although companies have implemented Occupational Safety and Health (OSH) programs, unsafe behaviors continue to occur due to differences in workers' levels of knowledge and compliance with established safety procedures. This condition highlights the importance of understanding the role of OSH knowledge and OSH compliance in shaping workers' behavior. Therefore, this study aims to describe the influence of OSH knowledge and OSH compliance on unsafe work behavior among employees at PT. Bhumi Phala Perkasa. This study employed a qualitative approach using a descriptive research design. Informants were selected through purposive sampling, consisting of workers as the primary informants, while the Supervisor, Production Manager, and HSE Officer served as supporting informants. Data were collected through in-depth interviews, observations, and documentation. The collected data were analyzed using the stages of data reduction, data presentation, and conclusion drawing. Data trustworthiness was ensured through source triangulation, technique triangulation, and time triangulation. The findings revealed that most workers possessed a good level of knowledge regarding workplace hazards, the use of personal protective equipment (PPE), and OSH procedures implemented within the company. However, compliance with OSH practices was not consistently maintained, resulting in the continued occurrence of unsafe work behaviors. These behaviors included the incomplete use of PPE, failure to comply with established work procedures, and a tendency to take shortcuts to complete tasks more quickly. The study concludes that unsafe work behavior is influenced by workers' level of knowledge, compliance with OSH procedures, work habits, the effectiveness of supervision, and the prevailing safety culture within the organization. Therefore, future research is recommended to explore safety culture and safety leadership in greater depth by involving a broader range of informants to obtain a more comprehensive understanding.
