Abstract

Cervical cancer remains a major public health problem in Indonesia and constitutes the second most common cancer among women in North Sumatra. The World Health Organization has set the 90-70-90 cervical cancer elimination target by 2030, in which 70% of women aged 35 and 45 must undergo high-performance screening. HPV DNA testing using self-sampling has emerged as an innovative strategy to increase screening coverage, particularly in areas with limited access such as Medan. This literature review aims to synthesize scientific evidence regarding the effectiveness, acceptability, implementation barriers, and opportunities of HPV DNA self-sampling to strengthen cervical cancer screening programs in Medan. The review followed PRISMA 2020 guidelines using narrative synthesis. Literature searches were conducted across PubMed, Scopus, ScienceDirect, SpringerLink, Wiley, ProQuest, and Google Scholar for articles published between 2021 and 2026 using Boolean combinations of the keywords cervical cancer, HPV DNA, self-sampling, nursing, and screening. Of 1,248 articles identified, 32 met the inclusion criteria for narrative synthesis. The results demonstrate that HPV DNA self-sampling has diagnostic sensitivity comparable to clinician sampling (relative sensitivity 0.96-1.02), high acceptability (78-94%), and effectively increases screening participation among women who have never or rarely been screened. Nurses have a strategic role as educators, counselors, coordinators, and community health promoters in program implementation. The main barriers include limited health literacy, cultural stigma, distribution logistics, and integration with primary health services. This review concludes that HPV DNA self-sampling is a feasible and evidence-based strategy for accelerating cervical cancer elimination in Medan and provides substantial implications for strengthening community nursing practice.
