Dementia is a leading cause of disability among the elderly, and physical activity has been recognised as one of the modifiable risk factors for cognitive decline. Institutionalised older adults are particularly vulnerable because of restricted mobility and monotonous routines, yet evidence from Indonesian foundation-based care settings remains limited. Objective: This study aimed to analyse the relationship between physical activity and the level of dementia among older adults living at Yayasan Sosial Nelly Huang Indonesia, Medan. Methods: A quantitative analytic observational study with a cross-sectional design was conducted in January 2026. Forty-two elderly residents were selected through purposive sampling from a population of 71 using the Slovin formula. Physical activity was assessed with the Physical Activity Scale for the Elderly (PASE) and cognitive function with the Mini Mental State Examination (MMSE). Data were analysed using univariate distribution and the Spearman rank correlation test after a Kolmogorov–Smirnov normality check. Results: A significant relationship was found between physical activity and the level of dementia (r = 0.457; p = 0.002), indicating a moderate association in which more favourable levels of physical activity accompanied better cognitive status. Conclusion: Physical activity is significantly associated with the level of dementia among institutionalised older adults. Structured and sustained physical activity programmes are recommended as a nonpharmacological strategy to preserve cognitive function.
